Hello, I've been seeing the downcoding (as we all have) on E/M billing and while I have started to do appeals, I am wondering if there is anything I should add to the requests to get them processed at the higher level? Medical records are being sent, but is there anything else I should review? Is ICD-10 coding just as important? How are you all getting these approved for the correct payment?

Hello all, BCBS Michigan (not sure about other states) is supposedly phasing in major changes to incident-to-billing starting September 1st. I believe we must start using SA modifier for nurse practitioners. Has anyone else heard of this? We are just now starting to bill for a nurse practitioner and was looking for any guidance someone may have. Any classes or videos for billing nurse practitioners would also be appreciated. Hello, I have been reading through the posts for attempted IUD placements, but I couldn't find an exact answer. Our doctor tried to place the IUD and could not due to patient's uterus. My question is if I can bill for the IUD device? My thought was No as she did not leave with one. But looking online some answers said to first bill the insurance and then if denied ask the manufacturer for a replacement. Good morning, The ER doctor has documented hematemesis with melena and nausea and vomiting. In the Medical Decision Making he also documents "esophagitis". My question is would you code esophagitis with bleeding since hematemesis is documented and take off the code for hematemesis or esophagitis unspecified and leave the hematemesis coded as is. Seems logical to code esophagitis with bleeding, but again, he specifically does not say with bleeding. Thank you for your help.
