FDA in USA Approves Besremi for Adults With Essential Thrombocythemia Diana Ernst, RPh | Publish DateAugust 31, 2026 Ropeginterferon alfa-2b-njft demonstrated a statistically superior durable clinical response over anagrelide in patients with hydroxyurea-resistant or intolerant essential thrombocythemia. The Food and Drug Administration (FDA) has expanded the approval of Besremi® (ropeginterferon alfa-2b-njft) to include the treatment of adults with essential thrombocythemia, a rare blood cancer characterized by the overproduction of platelets. Besremi, a long-acting interferon alfa-2b therapy, previously received approval for the treatment of polycythemia vera in 2021. Approval in patients with essential thrombocythemia was supported by data from the phase 3 SURPASS-ET trial (ClinicalTrials.gov Identifier: NCT04285086). Study participants with documented resistance or intolerance to hydroxyurea were randomly assigned to receive ropeginterferon alfa-2b subcutaneously every 2 weeks (n=91) or oral anagrelide daily (n=83). The primary endpoint was durable clinical response measured using modified European Leukemia Net (ELN) criteria at months 9 and 12. Response criteria included a reduction in peripheral blood counts (platelets ≤400×10⁹/L and white blood cells <9.5×10⁹/L), an improvement or no worsening in disease-related signs (splenomegaly), and no bleeding or thrombotic events. Findings showed treatment with ropeginterferon alfa-2b led to a significantly higher response rate
